You can say “我卡丢了，想要挂失. (wǒ kǎ diūle, xiǎng yào guàshī.)” Here are some more materials for you to learn to speak Chinese on this topic: 需要填挂失单吗？（Xūyào tián guàishī dān ma?） Do I need to fill out a loss report? 我能补办一张卡吗？(Wǒ néng bǔ bàn yì zhāng kǎ ma?) Can I register a new card?
